Texas Kids First -Supplemental Insurance

Supplemental Accident Insurance

The district has purchased student insurance for all students for accidents that occur at school or at school activities. Coverage also includes travel to and from such games and practice sessions.
Please see the attached claim form.  If a student is injured at school, the district (teacher, sponsor/coach, nurse, or principal) needs to complete the top (section 1) of the form and sign and the parent/guardian needs to complete the bottom portion (Section 2).  KEEP a copy of the form and send a copy to Angie Price. Submit the form to the insurance company - directions on the form.

After the insurance company receives the completed Student Accident Claim Form, it will send a letter to the parent/guardian letting them know they have received it.
